1. Application Form
The form is available on our website and in the School. In point 22 fill in the table with marks for all your exams and classes from your last four study semesters. For example, if your marks are: 4.0, 5.0, 4.0, 3.5, 4.0, 5.0 you should put down 5.0 x 2, 4.5 x 0, 4 x 3, 3.5 x 1, 2.0 x 0.(Attention: if in the candidate's country there is a different system of grading, the candidate is requested to describe it on an additional sheet of paper and not to fill in point 22).

2. Letter of Intent
explaining why you would like to study at the School (it should not exceed one typewritten page), in two language versions - in Polish and in English. The recruiting committee will read this letter very carefully.

3. An Authenticated Copy Of Your M.A. Diploma (original)

4. An Authenticated Copy Of Your Grades
over the last two years of your studies. This should be issued by your Dean's office or registrar. It can be an authenticated computer printout.

5. Document confirming AT LEAST FIVE YEARS of your full-time work experience e.g.:
a certificate from your employer (copy + original to compare at the moment of documents submission),
a certificate proving you have run or run your own business (original)

6. Five passport photographs - (please, write your name on the back of your photos in pencil).

7. If possible, additional documents (originals): certificates proving completion of courses, letters of recommendation, diplomas, etc.
